This guide will walk you through everything you need to know to prepare for your first visit, from scheduling your appointment to meeting your care team.
Before Your Appointment (Scheduling and Registration)
To receive care at CWWCHC, you’ll need to register as a new patient. You can do this by:
- Calling our center at 704 393 7720 to schedule your first appointment
- Registering online for convenience
On the day of your visit, arrive 15 minutes early to complete your registration. This extra time ensures we can gather all necessary information before you see your care team.
What to Bring for Registration
To register efficiently, please bring:
A valid form of identification, such as:
- Driver’s license
- Employment or School ID
- Social Security Card
- Green Card
- Valid Passport
- Veteran’s ID
- Medical Card or Insurance Card (if applicable)
We accept most health insurance plans, including Medicaid and Medicare. If you have insurance, please bring your insurance card.
For Uninsured Patients
If you don’t have health insurance, don’t worry. We offer a Sliding Discount Fee Scale based on family income and size. To determine your eligibility, please bring:
- Proof of Income (paycheck stub, tax form, or bank statement showing direct deposits)
- Proof of residence (a bill addressed to you)
- Any evidence of health or dental insurance coverage (if applicable)

To provide you with the best care, our team will ask about:
- Your past medical history
- Any chronic conditions or ongoing treatments
- Your family medical history
If you have previously seen another doctor, specialist, or visited an emergency room, please bring any test results, care plans, or discharge summaries you have. If you need help transferring your medical records, let us know when you call, and we’ll connect you with our medical records department.
Medications Review
Bring all your current medications (both prescriptions and over the counter) so our care team can review them and ensure proper treatment.
