Founded in 1981, The C.W. Williams Community Health Center (CWWCHC) is a designated Federally Qualified Health Center (FQHC). FQHCs are community-based organizations that provide primary and preventive care services in underserved areas. They offer a sliding fee scale based on income and operate under a governing board that includes patients. As a community Health Center, CWWCHC is part of one of the largest systems of care in the country that serves millions of patients regardless of their ability to pay.
CWWCHC is proud to serve as your medical home which is described below. Every day, we work to improve our practice by offering patients enhanced services and increased quality care. At CWWCHC, we work in multidisciplinary care teams consisting of physicians, nurse practitioners, medical assistants, front desk and referral specialists, pharmacists, case managers, behavioral health counselors, and outreach specialists. We use evidence-based care to ensure that each patient receives quality services at each visit. Please be patient with us as we transform our building and our practices to better serve you.
